Spoke to Severnprint.

Mike was most apologetic - he received the CD-R and covering letter and just booked it through the system as normal without checking.

He'd quoted for an A4 insert on top of the usual mag, I'd pointed it out in my covering letter and included a pdf booking form for them to print from on the CD-R.
No booking form printed though!

My idea was to make it easier for members to fill in a ready booking form without them having to go to the phaff of photocopying the mag...oh well.

I asked Mike to ensure we didn't get invoiced for the non existent booking forms as they were on his quote!
